William L. Hurley Limited and its employees and contractors ("Company" or "We") respect your privacy and are committed to protecting it through our compliance with this policy. For us to deliver a meaningful customer experience, we need to gather information from your healthcare providers and you directly. The information that we gather from your healthcare providers is used to deliver surveys and content to you. We will not do anything else with the information we receive about you from your healthcare providers. The information we receive directly from you, or with your permission, we will use to analyze survey results and may publish aggregate survey information about your healthcare experience. While your healthcare information from your providers is your information, the information you provide in survey responses is owned by William L. Hurley Limited.

That being said; however, we take your survey responses and aggregate them with other survey responses for statistical analysis and publication of aggregate data results.

We strive to conform to the Council of American Survey Research Organizations' (CASRO) Code of Standards and Ethics for Survey Research as well as the Direct Marketing Association's (DMA) and the Federal Trade Commission's (FTC) Fair Information Practice Principles, and comply with applicable privacy laws in the United States and other countries. 

This policy describes the types of information we may collect from you or that you may provide when you visit our websites and our practices for collecting, using, maintaining, protecting and disclosing that information.

  • Information that you provide by completing and responding to our surveys on our Website and any other use you may make of our Website. We may also ask you for information when you report a problem with our Website.

We receive the following information about you directly from your healthcare provider or EMR/EHR vendors: Name, gender, email address, date of birth, date of visit to healthcare provider practice(s), name of healthcare provider and location of provider.

Usage Details, IP Addresses, & Cookies

As you navigate through and interact with our Website, we may automatically collect certain information about your equipment, browsing actions and patterns, including the following:

  • Details of your visits to our Website, including traffic data, location data, logs and other communication data and the resources that you access and use on the Website.
  • Information about your computer and internet connection, including your IP address, operating system, and browser type. The information we collect about your usage is saved as aggregate, statistical data, and does not identify any individual. It helps us to improve our Website and to deliver a better and more personalized service by enabling us to estimate our audience size and usage patterns as well as determine which browsers respondents are using.

Disclosure Of Your Information

We may and will disclose aggregated information about our users, and information that does not identify any individual, without restriction. The information you give us in your survey responses is saved as aggregate, statistical data, and does not identify any individual. Survey responses are provided to your healthcare provider and other healthcare providers via our dashboard product.

We may disclose Personal Information that we collect or you provide as described in this privacy policy in the following ways:

  • To our subsidiaries and affiliates.
  • To contractors, service providers and other third parties we use to support our business and who are bound by contractual obligations to keep Personal Information confidential and use it only for the purposes for which we disclose it to them.

The William L. Hurley Limited may occasionally conduct visitor surveys to better target content to our audience.

William L. Hurley Limited offers online registration for events, admissions information, courses, restaurant reservations and similar which ask the website visitor to voluntarily provide personal information. The website’s registration forms may ask website visitors for contact information (like their name and e-mail address), a unique identifying number (for use as passwords in our record system), and/or financial information.

Information Request Forms

William L. Hurley Limited uses information request forms that ask the website visitor to voluntarily provide personal information such as names, addresses, and phone numbers. Due to the large number of individuals that submit contact information to William L.  B. Aggregated Tracking Information

Usage tracking

William L. Hurley Limited may track website visitor traffic patterns throughout our websites or social media platforms. However, we do not correlate this information with data about individual visitors. William L. Hurley Limited does break down overall usage statistics according to a visitor’s domain name, browser type, and Multipurpose Internet Mail Extension (MIME) type by reading this information from the browser string (information contained in every website visitor’s browser).

A text file called a “cookie” may be placed in the browser files of a visitor’s computer. Although the cookie file itself does not contain personal information, it provides information about a particular visitor’s use of a website. A cookie file can not read data off a hard disk or read cookie files created by other websites.

The placement of cookies can be blocked by changing the browser setting pertaining to cookies. This setting can also be changed to provide notification when a cookie file is placed. Note that our website does not require cookies to be accepted in order to function.
